structure: the package lives under packages/shield

Adopt the packages/ hard constraint. The @perish/shield package — deno.json,
lib/, tests/ — moves under packages/shield/; the root becomes a deno workspace
plus the shared fmt config. docs/ stays at the root (repo docs, not the package).
Identity unchanged; 0.1.0 stays live, the next release publishes from the new
path over the workspace.

Path updates: negentropy scan glob, release metadata version read, and guard's
check/test/purity targets (the purity loop reads packages/shield/lib now).
Verified by the guard, including the publish dry-run.
