release: keel family 0.4.1 ... 4cac9bf5dc

Carries the pg revival fix (#108) to the fleet. ensign pins keel from the
perish registry, so a landed fix reaches nothing until it is published —
ensign on main still cannot survive a postgres restart.

Compatible slot rather than 0.5.0: revive() is a defaulted trait method,
so no existing Wire impl breaks, and the only two in existence are
sqlite and pg, both in tree.
